Future CPOC Account Registration Page

For CPOC's future instance in CACWT, there are some data fields I'd like to edit on the Account Registration page.

Will it be possible to edit the following:
  1. "Select Agency" - Need to add Probation/CPOC to the dropdown menu
  2. "Department" - Can we create a dropdown menu here instead of having it be free form as it is currently? This will allow us to keep consistency in the data
  3. "Date of Hire" - Can we remove?
  4. "Date of Transfer" - Can we remove?
And can we add the following:
  1. "Create Username" - to open this up and encourage users to use a username they will remember that is NOT their current email address?
  2. "Email address" and "Secondary Email" (secondary as an optional field - in case they transfer departments or counties and need to update this information.
  3. Training Manager or Supervisor – allow user to indicate.. OR update AREA OF ASSIGNMENT to a dropdown of roles and have Training Manager/Supervisor be one of the options there.
  4. Create a drop down for "Affiliation" and have users select "Juvenile Probation"
